In the Order Notes / Special Instructions box in your cart, please include:

• Line 2: New last name or chosen name
• Line 3: Club name or theme — Wedding Club, Girls Club, etc.
• Line 4: Date or milestone — EST. 2026, wedding date, etc.
• Sweatshirt color
• Thread color preference
• Placement preference — center chest or left chest/pocket area
